Influence of protympanum pathology on the results of surgical treatment of patients with acquired middle ear cholesteatoma

ABSTRACT
The article analyzes the state of the protimpanum in various types of acquired cholesteatoma in adults and children. It was found that the spread of cholesteatoma in the protimpanum is observed for all types of acquired cholesteatoma, excluding the sinus cholesteatoma, and pathological changes in the mucous membrane of the protimanum and the bony part of the auditory tube in the form of polyps, granulations – only with primary acquired cholesteatoma. The influence of the pathology of the protimpanum on the choice of the type of surgical intervention and the method of rehabilitation of this middle ear section is noted. According to the study, the frequency of formation of residual cholesteatoma in the group of patients older than 15 years was 14,5%, including 0,9% in the protimpanum, and recurrent – 5,5%. In pediatric patients (from 3 to 15 years), residual cholesteatoma was detected in 18,7% of cases, including 1,9% in the protimpanum, and recurrent in 8,9%. A combination of risk factors such as the condition of the tympanic orifice of the auditory tube (its obstruction and / or anatomical constriction) and closed surgery technique predicted the development of recurrent cholesteatoma in both age groups. Pathological changes in the protimpanum did not affect the incidence of residual cholesteatoma. The key risk factors for the residual process were the type of cholesteatoma and the type of surgical intervention.
