Ultrasound picture of palatine tonsils in various forms of chronic tonsillitis

ABSTRACT
The problem of diagnosis of chronic tonsillitis (CT) remains relevant to this day. Despite the search for various diagnostic methods, when making a diagnosis and determining the severity of CT, we continue to rely on complaints, anamnesis, and examination data. One of the most common methods of instrumental diagnostics in medicine is ultrasound, which has a number of advantages: it does not require prior preparation, noninvasive, affordable, safe for pregnant women and children, and cost-effective. To date, ultrasound of the palatine tonsils (PT) is used in emergency hospitals for the differential diagnosis of paratonsillitis and peritonsillar abscess. The presented article examines the informative value of ultrasound in various modes (B-mode, color doppler mapping (CDM), and ultrasound compression elastography (USEG)) for the diagnosis of various forms of CT. The study involved 30 patients (6 men and 24 women), who were divided into 3 groups according to the form of CT: group I, simple form (n = 10) patients who had no effect after 2–3 courses of conservative therapy; group II, toxic allergic form I degree (TAF I) (n = 10) patients who had no effect after 1–2 courses of conservative therapy; group III, TAF II (n = 10). As a result of the study, it can be concluded that patients with different forms of CT differ statistically significantly (p ≤ 0.001) in shape, contour clarity, echogenicity, and vascularization of PT. And the total score of the PT assessment by ultrasound can serve as a predictor of the severity of CT. It determines TAF II with an accuracy of ROC-AUC = 0.858.
