ABSTRACT
Abstract. Objective. To present our first experience of using a titanium angular clip prosthesis for the reconstruction of the ossicular chain in long crus defects of the incus included 5 clinical cases. Of the 5 patients, three had a history of acubarotrauma, and one patient was admitted with a clinical picture of partial denture extrusion. On examination, the tympanic membrane was intact in two cases, cicatricial changes were present in one case, and a retraction pocket of the tense part of the tympanic membrane was present in the other case. The results of pure tone threshold audiometry in all patients showed conductive hearing loss with an air-bone interval (ABI) at frequencies of 500-1000-2000-4000 Hz from 20 to 40 dB. According to the computed tomography of the temporal bones, only two patients showed signs of destruction of the long crus of the incus. The patients underwent revision of the tympanic cavity, during which destruction of the auditory ossicular chain was noted in two cases at the level of the incus-stapedius joint with cicatricial fusion of the head of the stapes and the long crus of the incus and in three cases with carious changes in the shortened long crus of the incus. In four cases, a 2.25 mm long titanium angular clip prosthesis was installed, in one case, canaloplasty was required to visualize the incus and install a 3.25 mm long prosthesis. The audiometry results 6 months after the operation showed a reduction in air–bone gap at frequencies of 500-4000 Hz to 5-15 dB. Signs of prosthesis extrusion were not noted. Results. Using an angular titanium clip prosthesis is an effective way of bridging the incus and stapes in case of limited defects of the long crus of the incus. Conclusions. Despite the technical difficulties in installing the prosthesis, further development and evaluation of the remote results of this technique is a promising direction in the reconstruction of the ossicular chain.
