Determination of the depth of electric current passage through the bone tissue of fallopian canal in the process of needle electromyography stimulation

ABSTRACT
The objective of this research is to improve the safety of surgery of patients with the middle and inner ear pathology by preventing the damage to the facial nerve by conducting intraoperative monitoring of the facial nerve by the method of needle electromyography with continuous stimulation with a drilling machine. The clinical part of the prospective work was carried out on 48 patients that were diagnosed with suppurative otitis media. All the patients underwent surgery with the facial nerve intraoperational monitoring. After the surgery with the intraoperative monitoring, the facial nerve with the intact bone wall was stimulated electrically in the potentially dangerous damage areas. Minimum (threshold) stimulation (mA) of the facial nerve with a threshold event of 100 µV was used to register EMG events. The anatomical part of the work was carried out on 30 unformalinized cadaver temporal bones from adult bodies. The study of the thickness of bone wall of the fallopian canal tympanal and mastoid portions of the cadaver temporal bones was carried out for comparative analysis of the obtained data with intraoperative results of the threshold transosseous facial nerve simulation (vA). The statistical analysis of obtained data was carried out with parametric methods (Student’s ttest), non¬parametric correlation (Spearman’s method) and regression analysis. In the course of the comparative analysis of the fallopian channel bone wall thickness (mm) obtained as a result of the cadaver temporal bones dissection, with the data of the threshold transosseous facial nerve simulation (mA), perofrmed upon completion of the radical surgery in adult patients with intact fallopian canal bone wall, we determined the approximate depth (mm) of location of the facial nerve situated in the bone canal by means of the facial nerve intraoperational monitoring. It was found that 1 mA of threshold amperage corresponded to 0.8 mm thickness of the bone wall of the facial canal.
