Conservative combined treatment of primary regional larynx cancer using neoadjuvant target therapy

ABSTRACT
The objective of the study is to improve the efficacy of combination treatment of the patients with regional larynx cancer using conservative operations, neoadjuvant palliative chemotherapy in combination with target preparations. Methods: the authors have analyzed the results of treatment of 75 patients with regional squamous cell larynx cancer, divided into three groups. Polychemiotherapy was provided by the following preparations: cisplatin, fluorouracil, cetuximab. During the surgical intervention, the patients were performed simultaneous reconstruction with the greater horn of thyroid cartilage for three-dimensional replacement of posterior larynx defect and the possibility of further rehabilitation of the dividing function of larynx. Results: the authors have assessed oncological and functional results of the treatment. The best results were observed in the group with the expressed sensitivity of the tumor cells to chemiopreparations, where the tumor resorption and degree 3-4 medicinal pathomorphosis were observed. The inclusion of neoadjuvant palliative chemotherapy with EGFR antagonist into the plan of combined treatment of the patients with regional larynx cancer makes it possible to achieve radical and, at the same time, function-sparing operations.
