Cochlear implantation in patients with chronic otitis media

ABSTRACT
Abstract. Patients with profound hearing loss up to deafness and chronic otitis media are promising candidates for cochlear implantation, but they require attention from surgeons. Despite the relatively small number of patients with a combination of chronic otitis media and profound bilateral sensorineural hearing loss, this group is extremely heterogeneous and often requires an individual approach. The study carried out at the Saint Petersburg ENT Research Institute from 2014 to 2024 included 32 patients with profound bilateral sensorineural hearing loss and chronic otitis media. Two-stage tactic of managing this group of patients was chosen in cases of granulating polyps and retractions in the tympanic cavity. First, tympanoplasty was performed, then 6–12 months later cochlear implantation was carried out. Patients who had a defect in the pars tensa of the tympanic membrane in non-inflammatory period underwent simultaneous tympanoplasty and cochlear implantation. All patients underwent otomicroscopy 1 month and 6 months after surgical treatment. In all cases except one, in the long-term postoperative period, complications were not observed. It is necessary to develop an algorithm of surgical tactic and management of such patients, which allows determining the stages of interventions on the middle ear and cochlear implantation, minimizing the risk of complications, and increasing the effectiveness of auditory and speech rehabilitation.
