Morphological confirmation of possibility of preserving functional tissue below cryptal zone in pharyngeal tonsil in cold plasma dissection

ABSTRACT
Abstract. The development of surgical techniques for destruction of pharyngeal tonsil tissue by physical factors and modern equipment for adequate visualization of the surgical area make it possible to optimize surgical techniques for intervention on adenoids. Using morphological assessment of samples from severed cryptal zone of adenoids and the nasopharyngeal wall in the area of the surgical wound after cold plasma dissection of adenoids, the technical possibility of partial dissection of adenoids at the level of the cryptal fundus and the presence of functional tonsillar tissue of the base of the pharyngeal tonsil below the crypts was demonstrated. It was shown that the preserved in the nasopharyngeal arch amygdala base tissue contains lymphoid follicles; the density of lymphocytic infiltration of the adenoid base tissue under the crypts and in the cut-off adenoid lobule is identical; the neutrophil content in the preserved adenoid base is significantly lower than in the cryptal zone. The demonstration of the presence of true lymphatic tissue (follicles) below the cryptal part of adenoids forms the basis for the implementation of an organ-preserving approach in pharyngeal tonsil surgery by cold plasma dissection. The found features of cellular infiltration demonstrate the topical features of the intraorgan inflammatory process in adenoiditis, which can be used as a morphological justification for partial surgical intervention on adenoids in a state of chronic inflammation.
