Serum periostin is biological marker of polypous rhinosinusitis progression

ABSTRACT
Abstract. Despite the existence of various treatment regimens, control of chronic polypous rhinosinusitis (PRS) remains insufficient, which significantly worsens the quality of life of patients and imposes a persistent financial burden. With the progression of the disease, significant changes in biochemical parameters occur long before the appearance of clinical and subjective signs. In this connection, the availability of a biological marker, which can serve as an indicator of the progression of the pathological process and the pharmacological response to therapeutic intervention, will be of great importance for taking timely preventive measures. According to medical research, the best single predictor of Th-2–type inflammation activity is periostin, a protein involved in the process of subepithelial fibrosis. However, the possibility of its use in order to predict the course of PRS and control therapeutic responses requires further study. Objective. To determine the quantitative content of periostin in the blood serum of patients with PRS with a predominant Th–2 endotype of inflammation at different clinical stages of the polypous process. Patients and methods. Three groups of patients with PRS of allergic genesis were selected in accordance with the clinical stage of the disease (stage 1: polyps limited to the middle nasal meatus, stage 2: polyps extending beyond the middle nasal meatus, stage 3: polyps obstructing the corresponding half of the nose). In each patient, the concentration of periostin in the blood serum was determined by enzyme immunoassay. The difference in the content of the biomarker between the groups turned out to be statistically significant, its concentration correlated with the stage of the disease. The progression of the disease was accompanied by a statistically significant increase in the periostin content in the blood serum. This allows us to consider the level of serum periostin as a reliable non-invasive biomarker of the progression of the polypous process in the nasal cavity.
