ABSTRACT
Abstract. Antistreptolysin O (ASO) remains a widely used marker of streptococcal infection; however, its diagnostic and prognostic value in rheumatic diseases continues to be a matter of debate. Objective. To analyze current data on the relationship between chronic tonsillitis, streptococcal infection, and rheumatic diseases, as well as to assess the clinical significance of ASO. The structure of the review includes a characterization of the pathogenetic mechanisms and ASO dynamics, an analysis of its diagnostic value in post-streptococcal complications, and an evaluation of the role of ASO in patients with rheumatic diseases and immunosuppression. It has been shown that a single ASO measurement does not always reflect active infection, and its interpretation requires consideration of titer dynamics, age- and region-specific reference values, as well as comparison with other serological indicators. Some studies confirm the importance of ASO in the diagnosis of chronic tonsillitis in rheumatic diseases, while others highlight the limitations of its use. It is also important to consider that variability in the immune response may significantly influence the interpretation of obtained results in various clinical settings. The need for a comprehensive, multidisciplinary approach and for further research aimed at clarifying the prognostic value of ASO in rheumatic diseases has been emphasized.
