ABSTRACT
Abstract. Treatment of polypous rhinosinusitis remains a complex problem, despite a long history of using surgical and medical methods. The study was conducted to compare the effectiveness of the local application of a recombinant interferon (IFNa2b + IFNy) composition as an adjunct therapy to intranasal corticosteroids (InGCS) in the treatment of patients with polypous rhinosinusitis. Patients and methods. A prospective randomized trial included 90 patients with polypous rhinosinusitis (32 men, 58 women, mean age 53 years). The patients were divided into three groups (n = 30): 1st — intranasal monotherapy with a composition of interferon monotherapy ?2b and ? (INF), 2nd — a combination of (IFN?2b + IFN?) + InGCS, 3rd — InGCS monotherapy. The course of treatment is 1 month. Further, patients in all groups were transferred to basic therapy with intranasal glucocorticosteroids. The assessment was performed before treatment, after 1, 6, and 12 months using the SNOT- 22 questionnaire, olfactometry, endoscopic scale of nasal polyps, rhinorrhea, and IL-8, IL-1b in nasal secretion. Results. All three schemes led to a significant improvement in quality of life (SNOT-22) and a decrease in the IL-8/IL-1? ratio (p < 0.001). The combination of INF + InGCS significantly exceeded InGCS monotherapy in olfactory restoration (15.5±2.4 vs. 10.1±1.8 after 1 month, p < 0.001; 17.4±2.8 vs. 8.8±2.0 after 12 months, p < 0.001) and reducing IL-1? in nasal secretions (52.0 vs. 65.5, p < 0.001). Monotherapy with a combination (IFN?2b + IFN?) was not inferior to InGCS for the main parameters. Conclusion. The proposed intranasal interferon composition (IFNa2b + IFNy) with a transport medium has an efficacy comparable to InGCS as monotherapy and provides additional advantages when combined with InGCS, especially regarding olfaction. The results obtained substantiate the expediency of further studies of intranasal interferons in polypous rhinosinusitis.
