ABSTRACT
Abstract. Nasal septum surgery is one of the most common operations performed by otorhinolaryngologists for more than 140 years. Nevertheless, issues related to complications, both intraoperative and postoperative, remain open. Objective. To determine the frequency and structure of intraoperative defects of the nasal septum mucosa during septoplasty and identify the prerequisites for the formation of this complication. Patients and methods. The study involved 115 patients out of 746 (15.4%) with the intraoperatively ruptured or dissected mucous membrane (MM) of the nasal septum, with a length of more than 0.5 cm. All patients were admitted for elective surgery with a diagnosis of the nasal septum deviation for septoplasty or simultaneous intervention, if necessary. In order to determine the condition of the nasal septum in the preoperative period, the severity of the nasal septum ridge as well as mucosal atrophy was assessed. For this purpose, a visual analog scale (VAS) in points was used. The statistical computing environment R 4.1.3 was used for mathematical data analysis. Results. When analyzing the size of intraoperative defects of the MM of the nasal septum, defects with a length of 1 to 2 cm were most often observed in 38 patients (33%). The occurrence of defects up to 1 cm and from 2 to 3 cm was determined with equal frequency in 30 patients (26%). The least common (17 patients, 15%) were intraoperative defects of more than 3 cm. Defects of MM were detected in patients with a curvature in the form of a ridge in 62.6% of cases of all nasal septum deformities. Mucosal atrophy by 3 points was determined in 97.3% of patients in this group. Conclusion. The frequency of intraoperative defects of the nasal septum mucosa during septoplasty was 15.4%, and in 62.6% it was due to pronounced ridges, and pronounced atrophy from the nasal septum in them was 97.3%, which can cause an intraoperative defect even with careful dissection of the mucous membrane.
