ABSTRACT
Abstract. Objective. Тo substantiate the feasibility of identifying a form of otitis media with effusion with isolated mastoid opacification based on clinical and audiological indicators and radiological data. Patients and methods. The study involved 22 people aged 6 to 47 years with atypical manifestations of otitis media with effusion. A comprehensive examination included audiological research methods (acumetry, pure tone threshold audiometry, and objective diagnostic methods) and radiological diagnostics. Results and discussion. It was revealed that disturbances in the audiological profile in patients with isolated mastoid opacification in cases of otitis media with effusion were characterized by a predominance of pathological changes in acoustic reflexometry and otoacoustic emissions, minimal deviations in pure tone threshold audiometry, and the absence of changes in tympanometry. In all patients of the studied groups, during radiological diagnostics, there was an isolated mastoid opacification in the form of total/subtotal darkening or decreased pneumatization in the area of the mastoid apex. Analysis of the results of a clinical-audiological study and computed tomography (CT) diagnostic data allows us to regard the identified isolated mastoid opacification as manifestations of otitis media with effusion, even in the absence of changes in the otoscopic picture and tympanometry and acumetry data. Conclusion. The clinical and audiological features identified during the study in combination with isolated disorders of pneumatization of the mastoid process during CT examination should be considered as atypical manifestations of otitis media with effusion requiring appropriate treatment tactics.
