Incidence rates of acute mastoiditis in children and its complications in West Siberian plain of Tyumen region

ABSTRACT
Abstract. Introduction. Acute mastoiditis, being a frequent complication of acute otitis media in children, remains an urgent issue in otolaryngology. In this context, increasing knowledge about the development of otogenic complications in relation to the demographic and geographical living conditions of children will, in a certain sense, help doctors to provide highly specialized otolaryngologic care earlier. Objective. To enhance the early diagnosis, treatment, and prevention of acute mastoiditis associated with acute otitis media and its complications in childhood, based on the study and assessment of the contribution of demographic and geographical factors to the development of the disease. Patients and methods. A retrospective study of children admitted to Tyumen Regional Clinical Hospital No. 2 with various complications of acute mastoiditis associated with acute otitis media for the period from 2012 to 2023 was performed. Results and discussion. Of the 136 patients with acute mastoiditis, 70% (95 patients) had intratemporal complications, 30% (41 patients) had intracranial complications; 62% of boys and 38% of girls. The average age with intratemporal complications was 5.5±4.4 years, with intracranial complications 6.3±4.1 years. The relationship between demographic variables and mastoiditis was revealed. Younger age groups, especially children aged 0-1 years (OR: 3.9; 95% CI: 1.05–2.77), and at 3-6 years showed the highest chances of developing acute mastoiditis (OR: 2.3 95% CI: 0.34–0.89), there were no gender differences. Children living in Tyumen was associated with an increased risk of developing mastoiditis — OR: 1.4, in other cities of OR: 1.5; while in rural areas it is significantly lower (OR: 0.4; 95% CI: 0.2–0.9), seasonal dependence is noted in summer (OR: 1.0; 95% CI: 0.61-1.65) and autumn (OR: 1.11; 95% CI: 0.69-1.78). It was confirmed that the duration of the disease increases the risk of mastoiditis (OR: 1.75; 95% CI: for 8-14; OR: 1.94; 95% CI: for 15-30 days). Conclusions. Increasing knowledge about the influence of demographic and geographical factors in the development of acute mastoiditis in children contributes to early diagnosis, adequate treatment, and prevention of intracranial and intratemporal complications.
