Exudative otitis media in children with congenital cleft lip and palate

ABSTRACT
Exudative otitis media (EOM) in children with congenital cleft lip and palate (CCLP) is the most frequent concomitant ENT pathology and the cause of hearing loss which significantly affects the child’s learning ability and intellectual development, the formation of child’s speech, social adaptation and life quality. The number of children with maxillofacial pathology is increasing, therefore, the problem is rather urgent. The basis for the development of EOM in patients with CCLP is the programmed auditory tube (АT) dysfunction – the inability of m. tensor veli palatini to open the auditory tube when swallowing, associated with CCLP. The additional factors contributing to EOM in CCLP are: the hypoplasia of the auditory tube, anatomical features of the cranio-facial skeleton, failure of the barrier function of the palate, sphincter mechanism of swallowing, pathological reflux of liquid and food into the nasopharynx. The complexity of this congenital abnormality dictates the need for an interdisciplinary approach of various specialists to the problem of rehabilitation of patients with congenital maxillofacial pathology.
