ABSTRACT
Epidemiological studies of upper respiratory tract diseases in children showed that allergic rhinitis is the predominant pathology, it covers 11.7% of children in the Russian Federation. The role of non-allergic rhinitis in the structure of chronic rhinitis in children and the specific features of its etiology and pathogenesis are still being studied. Immune inflammation in non-allergic rhinitis is considered to play the dominant role. The objective of this work was to study the efficacy of decreasing the local inflammatory process in children with allergic and non-allergic rhinitis (immunoinflammatory nosology) during topical application of mometasone furoate. The authors examined two groups of school-age children: 48 children with allergic rhinitis and 37 children with non-allergic rhinitis, comparable in terms of age and gender composition. All children were administered NozefrineR (mometasone furoate) for one month, 50 mcg into both nasal passages once per day. The clinical manifestations of rhinitis, the study of the concentrations of cytokines IL-4, IL-1b, TNF-a, ING-g, INF-a in the nasopharyngeal wash and the cellular composition of the rhinocytogram were registered prior to the administration and 1 month after its termination. Clinical manifestations of rhinitis (nasal obstruction, rhinedema, rhinorrhea) disappeared in 73% of children with allergic rhinitis after administration of NozefrineR, the number of neutrophilic leukocytes in impression smears, concentrations of IL-4, TNF-a in nasopharyngeal swabs significantly decreased and the concentration of INF-g in it increased. The treatment of non-allergic rhinitis with NozefrineR resulted in the reduction of clinical manifestations of rhinitis (nasal obstruction, rhinedema, rhinorrhea) only in 46% of patients, but local inflammation significantly decreased due to suppression of the synthesis of IL-4, IL-1b, TNF- a, ING-g. The number of eosinophilic and neutrophilic leukocytes in impression smears of children with non-allergic rhinitis decreased after treatment with NozefrineR.
