ABSTRACT
Abstract. Objective. To increase the effectiveness of mastoidoplasty in patients with chronic suppurative otitis media. Patients and methods. 53 patients with a disease duration of more than 15 years were examined and treated, 32 (60.4%) of them had previously undergone open-type sanitizing surgery on the middle ear. All patients underwent antromastoidectomy with reconstruction of the posterior wall of the external auditory canal and the sound-conducting apparatus of the middle ear. In the 1st group (n = 17), the bone substitute material Histograft was used for mastoidoplasty; in the 2nd group (n = 13), a mixture of Histograft with autobone chips in equal proportions; and in the 3rd group (n = 23), autobone chips. Results. The results of the volume and shape of the external auditory canal were finally formed 3 months after the surgery. The advantage of using a mixture of the Histograft material and autobone chips was noted as it is more osseointegrable, not subject to migration in presence of exudation in the postoperative period. The volume of the external auditory canal (ECV) after 3 and 6 months in the 1st and 2nd groups averaged 1.64 cm3 (±0.77), in the 3rd group, 2.65 (±1.35). According to CT data, the diastasis of the material used with the bone wall of the cavity was <1 mm in 70.59% (group 1), 92.31% (group 2), and 73.91% (group 3). Conclusions. The use of the osteoplastic bone-substituting material Histograft as the main component and mixed with autogenous bone chips for obliteration of the antromastoid cavity with simultaneous reconstruction of the sound-conducting system is an effective method of treating patients with chronic suppurative otitis media, including after a previously performed open operation due to the restoration of the anatomical shape of the external auditory canal and improvement of hearing function.
