Case report of pleomorphic adenoma of parapharyngeal space

ABSTRACT
Abstract. Pleomorphic adenoma remains the most common benign tumor of the salivary glands. However, the presence of pleomorphic adenoma in the parapharyngeal space, either de novo or as an extension of the deep lobe of the parotid gland, remains rare in the practice of otorhinolaryngologists. Due to its benign nature, symptoms are usually mild or appear late, when the tumors have already reached a large size, which represents a true diagnostic and therapeutic challenge. Despite its benign behavior, the definitive treatment should be complete resection of the tumor. The choice of the surgical approach depends on various factors, including the exact location of the tumor, its relationship with neurovascular structures and its physical, histological, and radiological data. We present the case of a 73-year-old female patient who experienced symptoms for 3 years. The studies revealed the presence of a pleomorphic adenoma in the left parapharyngeal space. The patient underwent resection of the tumor using an open transoral approach, achieving complete removal of the tumor successfully, without the need for mandibulotomy or transcervical approach. Comprehensive knowledge of this complex anatomical area is vital as it helps make the right decisions regarding treatment and prevent patient mortality.
