Microbial composition and sensitivity to antimicrobial drugs of microorganisms isolated from washings from intubation and tracheostomy tubes

ABSTRACT
Abstract. Objective. To study the microbial composition and sensitivity to antimicrobial drugs of microorganisms isolated from flushes from intubation and tracheostomy tubes from patients treated in the intensive care unit. A study of the microflora of washings from 24 tracheostomy and 76 intubation tubes from 100 patients who were in the intensive care unit from October 2021 to October 2024 was conducted. The most common combination of gram-negative bacteria (Pseudomonas aeruginosa, Acinetobacter baumannii, Klebsiella pneumoniae) and yeast-like fungi of the genus Candida was established. The high frequency of allocation of Candida krusei in comparison with other species of this genus is noteworthy. In most cases, the quantitative results of swab culture ranged from 105 to 108 CFU. Strains of Staphylococcus aureus have been isolated from gram-positive bacteria. Frequently encountered resistance of Acinetobacter baumannii, Klebsiella pneumoniae, Pseudomonas aeruginosa strains to ampicillin, 3rd and 4th cephalosporin generation, and some aminoglycosides (gentamicin, amikacin) was revealed. Strains of gram-negative bacteria resistant to the aminoglycoside antibiotic tobramycin and carbepenes are increasingly being identified. Glycylcycline (tigecycline) was not effective against the studied strains in 10.7% (Klebsiella pneumoniae) and 13.5% (Acinetobacter baumannii, Pseudomonas aeruginosa). Strains with simultaneous resistance to 5 or more drugs were isolated in 38.5% (20 strains) of Acinetobacter baumannii, in 57.1% (16 strains) of Klebsiella pneumoniae, and in 45.5% (10 strains) of Pseudomonas aeruginosa. Preparations of combined bacteriophages were effective against 25–36% of the isolated strains, which makes it advisable to use these immunobiological drugs only in combination with other methods of antimicrobial therapy.
