Peculiarities of chronic rhinosinusitis phenotypes from standpoint of immune and cytological markers

ABSTRACT
Abstract. The work analyzes immune and cytological markers of various phenotypes of chronic rhinosinusitis (CRS). Objective. To identify the features of the phenotypes of chronic rhinosinusitis from the standpoint of immune and cytological markers. Materials and methods. A study of nasopharyngeal lavage cytokines (interleukin 1, interleukin 4, interleukin 6, and tumor necrosis factor) and a brush microbiopsy of the nasal mucosa using an original technique were conducted in 196 patients with various phenotypes of CRS. CRS with nasal polyps, CRS by allergic phenotype without nasal polyps, and CRS by neurovegetative (vasomotor) phenotype without nasal polyps were distinguished. Results and discussions. With the help of multiple linear regression, immunological and cytological predictors and protectors of various CRS phenotypes were identified. Logistic equations for calculating the diagnostic risk for each CRS phenotype were obtained using logit transformation. Conclusion. Of particular importance in the diagnosis of CRS phenotypes as well as in the pathogenesis of these various nosological entities are the concentrations of proinflammatory and proallergic cytokines in nasopharyngeal washes (in fact, their topical synthesis), such as interleukin 6, interleukin 4, and tumor necrosis factor alpha.
